Despite being widely used, there are still doubts and fears related to the functioning, safety and real effects of these drugs, which act directly on the blood flow of the penis. PDE5 inhibitors improve blood flow to the penis through a simple mechanism: they block the action of an enzyme that stops erections. The main contraindication is the concomitant use of nitrates, medicines for chest pain, as this combination can cause a sudden drop in blood pressure.

In some cases, such as tadalafil in a low daily dose, the medicine also improves urinary symptoms caused by an enlarged prostate and contributes to the vascular health of the penis. For men with low testosterone, the ideal is to simultaneously treat the hormonal disorder, as the PDE5 inhibitor alone may not be enough.
Medicines may not work well in situations such as decompensated diabetes, severe obesity, heavy smoking, serious vascular diseases or untreated sleep apnea. In these cases, an integrated approach, which includes lifestyle changes and metabolic adjustments, often significantly improves results.
